A 65-year-old woman sustained a complex peritrochanteric femoral fracture in April 2020, during the first COVID-19 lockdown. Limited access to definitive trauma care resulted in conservative management with traction. She presented approximately six months later with an established nonunion, marked displacement, malalignment and abundant callus formation. Despite the complexity of the fracture, the femoral head, proximal bone stock and trochanteric region remained sufficiently preserved to support a head-preserving reconstruction rather than arthroplasty.
