Carpal Tunnel Release

A 60-year-old woman presented with hand paresthesia, thenar and abductor pollicis brevis wasting, and positive Tinel and Phalen tests, consistent with advanced carpal tunnel syndrome. Concomitant findings included a symptomatic trigger thumb and a painful cystic swelling over the volar ring-finger MCP joint, clinically suspected to arise from the joint or flexor tendon sheath.

1. Under axillary block and tourniquet control at 250 mmHg
2. The trigger thumb was treated by central A1 pulley release
